Please read and initial each statement below:

If your present relationship/ living situation were to change and you were no longer able to care for the dog, a new application must be submitted and approved to transfer ownership.

I agree to provide my own collar, leash and a personal ID tag at the time of completing the adoption contract.

I understand and agree to pay the adoption fee.

NO REFUNDS OF DONATIONS AND FEES:

Donations and fees to rescue are irrevocable and non-refundable. If the Adopter returns the dog being adopted to Rescue for any reason, Adopter is not entitled to a refund of any part of any donation and/ or fee made by Adopter.

Adoption Fee Includes:

-    Sterilization
-    Health exam
-    DHPP (first set of vaccinations)
-    Bordatella
-    De-wormer
-    Flea and tick preventative
 

Rescue reserves the right to refuse adoption to any applicant for any reason. This questionnaire becomes part of our contract.
All adopted puppies and/ or dogs will not be relinquished to any entity (rescue, humane society, etc.) and only will be returned to Del Mar Dog Rescue.
